Demonstration sites

Med-HEART will collaborate with the nine demonstration sites listed below to evaluate models of care that link individuals who are homeless and living with HIV to needed services and resources:

  • AIDS Arms, Inc., Dallas, TX: Intensive care coordination intervention to link and retain individuals who are HIV+, multiply diagnosed and homeless in a medical home

  • Family Health Centers of San Diego, Inc., San Diego, CA: A collaborative peer navigator model for serving individuals in San Diego who are homeless, HIV-positive, and dually diagnosed

  • Harris Health System, Houston, TX: A medical home delivering HIV care at the site of choice for homeless individuals in Houston

  • Multnomah County Health Department, Portland, OR: Using network navigators to connect individuals who are multiply diagnosed, homeless, and HIV-positive with a medical home in Portland, Oregon

  • City of Pasadena Public Health Department, Pasadena, CA: An innovative mobile care project, providing vital care navigation and outreach to individuals who are homeless and HIV-positive in the San Gabriel Valley

  • San Francisco Department of Public Health, San Francisco, CA: The HOME Project: Building a medical home for people who are multiply diagnosed, HIV-positive, and homeless

  • CommWell Health, Newton Grove, NC: Patient-centered medical home for people with HIV who are homeless or unstably housed and have substance use and/or mental health diagnosis in rural North Carolina

  • University of Florida-UF Cares, Jacksonville, FL: A partnership for access to treatment and housing in Jacksonville, Florida

  • Yale University, New Haven, CT: The mHEALTH model creates a medical home for people who are homeless and living with HIV in New Haven, CT.
